You are invited to participate in a web or app based online Survey. This is a health questionnaire being conducted by Physitrack (also referred to below as “we” or “We”) for analytical use only in order to improve our Services.
We would remind you that we act as a technology provider only and are not responsible nor liable for any use or reliance on the platform or this Survey. This Survey (to include its results and our analysis of these) and our platform are not medical devices nor provide nor are intended to provide any form of medical advice, medical decisions, diagnoses or treatment whatsoever.
All data which you provide in the Survey is fully anonymised and when we analyse this data, we will not be able to identify whichever patient/end user who is completing it.
We together with any of our affiliated companies do not accept any liability regarding the information provided and cannot be held liable for any actions, losses, or damages resulting from the participation by you in the Survey.
Participation
Your participation in this Survey is voluntary. You may refuse to take part in the research or exit the Survey at any time without penalty. You are free to decline to answer any particular question you do not wish to answer for any reason.
