Continuity across NHS specialties and teams

MSK, community, neurological, respiratory, cardiac, persistent pain, older people's, women's and paediatric rehabilitation. The patient keeps one programme as it travels with them through the pathway, personalised throughout by the treating clinician.

Technical Assurance

“Can this be implemented safely and efficiently in our digital environment?”

  • Web-based access: no specialist desktop software installation required for standard use
  • Standard deployment requires minimal local IT infrastructure
  • Live TPP SystmOne integration; other integrations described only when current and verified
  • Physitrack supports Single Sign-On (SSO) with Microsoft Entra ID (formerly Azure Active Directory) using OpenID Connect (OIDC). This enables NHS organisations to manage authentication and access through their existing Microsoft identity environment, including the application of their own access and multi-factor authentication (MFA) policies. SSO requirements and configuration are agreed during implementation scoping. Two-factor authentication (2FA) is also supported using standard authenticator apps (setup guide)
  • Implementation, training, technical support and ongoing Customer Success engagement included

Physitrack is web-based and does not require specialist desktop software installation for standard browser access. Clinicians work on desktop or PhysiAssistant on compatible iOS and Android devices; patients use PhysiApp on web, iOS or Android, with printed programmes available where clinically appropriate. Physitrack has a live TPP SystmOne integration, and single sign-on, firewall allow-listing and local configuration are agreed during implementation scoping.

Information Governance & Security Assurance

  • DSPT approved; ISO/IEC 27001 certified
  • NHS data hosted onshore, AWS London; DPIA support available for local assessments

Physitrack is approved through the NHS Data Security and Protection Toolkit (DSPT) and provides structured DTAC evidence covering clinical safety, data protection, technical security, interoperability, and usability and accessibility. Physitrack maintains clinical risk-management documentation in support of DCB0129, and holds ISO/IEC 27001 certification for information security. Relevant NHS patient data is hosted onshore in the AWS London region. Physitrack has a dedicated Data Protection Officer, Information Security Manager and Clinical Safety Officer.

NHS organisations can complete NHS procurement by purchase order or through the NHS London Procurement Partnership, with annual and multi-year contracting options agreed according to scope.

NHS case study

Leicestershire Partnership NHS Trust: Virtual Falls Prevention

The Trust's Falls Prevention Service supports around 950 patients a year. Traditional face-to-face group delivery faced capacity, geographic and scheduling constraints, so the service piloted PhysiApp-delivered virtual falls prevention.

“Attending in person would simply be too much”, on why remote, PhysiApp-based delivery worked where a face-to-face group could not.

PhysiApp is used by patients of all ages with a wide range of conditions and comorbidities to support their rehabilitation. Our platform data shows patients aged 60 and over engage particularly well: they complete more exercise actions per week on average than younger cohorts, and retain their engagement for longer.

No, to both. Physitrack complements face-to-face care and enables hybrid pathways where appropriate. The treating clinician always remains responsible for assessment, prescription and review. It can also help services use digital rehabilitation to support appropriate patients between contacts, so clinicians can focus direct input where it's most needed.

Not currently. Referral and clinical triage remain with the NHS service. Physitrack supports the pathway from Waiting Well onward. We are also developing digital onboarding capabilities designed to support earlier stages of the patient pathway. Speak to our NHS team to discuss this work and our developing capabilities.

Yes. You can create PDF printouts of exercise programmes, including storyboard mode with clear images of each exercise. Printouts can also include educational materials and patient-reported outcome measures, and come with a QR code so patients can easily access exercise videos.

Yes. Physitrack can be accessed by a patient's caregiver, for example when working with children or others who need support with their rehabilitation.
